I find that whatever goal I'm trying to achieve be it weight loss or mass gain , I'll be very motivated for a month and start slacking quite quickly. The key for me to stay motivated , is to see numbers go up or down every week. Keeping track of what you eat and measuring and weighing myself every Saturday morning is what keeps me going.
When I have a disappointing measurement , I can usually still remember what I did wrong during the week, so I'll have more resolve not doing that again in the following week.

I think that's the key , you have to keep confronting that part of you that is always in denial , with cold hard fact : this is where you are now , this is how well or bad you performed this week, period.

Of course don't drive yourself crazy and do it every day , those results will fluctuate too much. I think weekly is best

I do agree that writing it down can keep you going. You will see your progress and what you are doing to get there right there in front of you - I have done this to loose weight and it has worked for me. The trick is, though, to get to the point where you want to be, and keep the good habits and not NEED to document everything to keep being healthy
